Well, it would be the latter, but Mail::ClamAV appears to be
abandonware. The most recent version (Feb 2007) is labeled
"unauthorized release" by CPAN, and the last release by the original
author was in 2005.
I suspect what you really want to do is start clamd and have
MailScanner run clamdscan rather than running clamscan. This should
achieve the desired effect of not reloading definitions on every scan,
and is probably at least part of the reason that Mail::ClamAV has gone
dormant.
